Parents who pre-register online at eAYSO.org as well as those who register in person must have the following documents on hand for Tuesday's registration event:

  • Two signed copies of the registration form.
  • A completed Macomb Township Waiver for each child being registered.
  • A small photo – wallet size is preferred but any photo that can be cut to wallet size or smaller is acceptable.
  • The child’s birth certificate or passport (or the your birth certificate, passport or driver’s license if you are 18 years old and registering for the U19 division). AYSO doesn't need to keep a copy, just to verify the birth date on the registration forms.
  • Payment – cash and checks are accepted at registration events. If you paid online, bring a copy of your receipt.

These documents must be brought to a registration event to complete the registration process, even if the fees were paid online with a credit or debit card.
